INTRODUCTION : La paraphényléne diamine (PPD) est fréquemment utilisée par les femmes dans certains pays (Maroc, Inde, Pakistan et Soudan) pour teindre en noir ieurs cheveux. La connaissance de sa toxicité systémique a conduit à une utilisation darts un but d'autolyse. Cette intoxication n'a fait l'objet que de peu de travaux. C'est la raison pour laquelle il a semblé intéressant de rapporter une série de 13 cas d'intoxications graves à la PPD, recensés dans le service depuis 1986.

OBSERVATION : La majorité des victimes sont des femmes (12 cas) agées de 15 à 35 ans. L'ingestion est faite dans un but d'autolyse (11 cas). La symptomatologie clinique comporte pour l'essentiel une détresse respiratoire aigué en rapport avec un
oedéme diffus du cou ayant nécessité soit une intubation trachéale (6 cas), soit une trachéotomie (7 cas) de sauvetage. L'examen clinique met en évidence des myalgies diffuses, un volumineux oedéme du cou, du plancher buccal et de la langue. Les urines sont noirâtres. Il existe une augmentation nette de la créatinine phosphokinase (CPK) dont les concentrations varient entre 2140 et 54.750 ui/l affirmant le diagnostic de rhabdomyolyse. L'augmentation de la créatinémie est nette chez dix malades. Une hyperkaliémie majeure est observée dans cinq cas. Une hypocalcémie profonde est notée chez quatre malades. L'évolution est marquée par l'amélioration de l'état respiratoire aprés régression de l'oedéme du cou sous corticothérapie. Cinq patients ont bénéficié de plusieurs séances d'épuration extrarénale; la durée de l'insuffisance rénale aigué (IRA) est toujours inférieure à vingt jours. Une patiente est décédée a l'admission lors de la
trachéotomie, et deux patientes sont décédées par choc septique à point de départ pulmonaire.

DISCUSSION : L'utilisation de PPD, est largement répandue au Maroc, d'où son utilisation de plus en plus fréquente dans un but d'autolyse. La symptomatologie clinique est stéréotypée. Elle débute par des myalgies intenses, puis apparait rapidement un
oedéme cervicofacial et pharyngolaryngé responsable d'une détresse respiratoire aigue : cet oedéme cellulaire est probablement le résultat d'un changement de perméabilité membranaire. (1) L'atteinte musculaire est constante et diffuse,
confirmant les affinités de la PPD pour le muscle : la fibre musculaire est brutalement nécrosée puis éliminée par les macrophages (2). L'IRA est observée chez la
plupart des malades (70%) : il s'agit d'une nécrose tubulaire aigué (3). Le mécanisme le plus plausible est une précipitation intratubulaire de déchets cellulaires et de myoglobine secondaires à la rhabdomyolyse. Le traitement préventif de I'IRA repose sur le maintien de la volémie et d'une polyurie aléaline. SYSTEMIC TOXICITY OF PARAPHENYLENE DIAMINE

INTRODUCTION: Paraphenylene diamine (PPD) is frequently used by women in certain countries (Morocco, India, Pakistan and Sudan) to dye their hair black. Awareness of its systemic toxicity has led its use for purposes of suicide. This poisoning has only been the subject of a few studies. This is why it appeared interesting to report a series of 13 cases of serious poisoning with PPD, observed in the department since 1986.

OBSERVATION: The majority of victims are women (12 cases) 15 to 35 years old. Ingestion was done for purposes of suicide (11 cases). The clinical symptoms essentially consist of acute respiratory distress with diffusion edema of the neck requiring tracheal intubation (6 cases), or an emergency tracheotomy (7 cases). Clinical examination shows diffuse myalgia, and voluminous edema of the neck, the floor of the mouth and the tongue. The urine is blackish. There is a clear increase in creatinine phosphokinase (CPK), the concentration of which varies from 2140 to 54,750 IU/L, confirming the diagnosis of rhabdomyolysis. Increase in creatinemia is clear in ten patients. Severe hyperkalemia is observed in five cases. Profound hypocalcemia is noted in four patients. The outcome is marked by improvement of the respiratory state after regression of the edema of the neck with corticotherapy. Five patients were given several dialysis sessions; the duration of acute renal failure (ARF) is always less than twenty days. One patient died upon admission during the tracheotomy, and two patients died of septic shock of pulmonary origination.

DISCUSSION: The use of PPD is widespread in Morocco, where it is used more and more frequently for purposes of suicide. Clinical symptoms are stereotypical. They begin with intense myalgia, then a cervicofacial and pharyngolaryngeal edema rapidly appears, responsible for acute respiratory distress: this cellular edema is probably the result of a change in membrane permeability (1). Muscle impairment is constant and diffuse, confirming the affinity of PPD for muscle: the muscle fiber is badly necrotized then eliminated by macrophages (2). ARF is observed in the majority of patients (70%): this is a result of acute tubular necrosis (3). The most plausible mechanism is an intratubular precipitation of cellular debris and myoglobin secondary to rhabdomyolysis. ARF preventative treatment relies on maintaining blood volume and alkaline polyuria. Fatal prognosis is possible due to the risk of asphyxia and hyperkalemia. After this episode, there is progression toward impairment of other organs.

CONCLUSION: The published series do not at all reflect the true frequency of this poisoning. Its seriousness should incite public authorities to intervene in the sale of this product.
