the point is, he is not in the bathroom, but in bed. Anna in fact asks if he is awake on the first line of the dialogue I gave. That's why I felt the more usual idiomatic sense didn't fit here

It can have a more "colorful" meaning, but I don't see it here. There's a man in the bathroom, probably taking his time so the woman asks him what he's doing (could she need to use the bathroom too?), and this is his way of saying he's almost done. Then he adds an open invitation that could lead to more, but I don't really think he's musturbating here, just peeing. Given how explicit Tuscan idioms can be (they can get away with things that would sound extremely rude said by anyone else), I doubt the lady would have replied "bischero" had he been up to something more... graphic :-)

The declaration of superiority/dominance contained in 'quello me lo sgrulla/sgrullamelo' could be related to the act of masturbation (i.e. obliging the opponent to masturbate the subject. Or, in other words, 'sgrullamelo' could be considered similar to 'suck my dick'). In a certain way as like as another slang expression, 'quello mi fa una sega' (that guy masturbates me = that guy is nothing compared to me) but I (so this is just a personal opinion) always thought that it refers more likely to the humiliation of obliging the opponent to act as the most miserable of the servants, that is the one which task is to shake off the pee drops off his master's dick. That is even a lower level (because is an act of lesser importance than sex) of the one involving masturbation.

I hope that all this complicate explanation is understandable.
Before I forgot to mention this because in the situation told by your text (a heterosexual couple) there could be a further meaning actually related to an ambiguity between clean my cock/masturbate me...

I really don't know why some people here doesn't aknowledge it...
BUT

it has an obscene meaning when, like in your case, is dick-referred.

I mean it is gross, rude. Generally, it involves masturbation.
And it is a very unpolite way to speak about it. At least! ;)

Also, from tuscan dialect it has been taken by Italian slang in saying like "sì al massimo quello me lo sgrulla"
to indicate that the one has to 'sgrullare' the speaker is somehow at a lower performance level (referred to everything: sport, study, general smartness, whatever) than the speaker itself.
