It's the name of the Joint Municipal Authority for North Karelia Social and Health Services, which literally means (as Pertti already suggested) "Your social and health services". It is not translated, but probably requires an explanation - depending on your context, of course. In any case, it's only specific to North Karelia region in Finland.

(your) social and health services (revamp)

The first word siun seems to be either a typo or else it is a dialectal form of sinun 'your'.

The second word sote is a relatively recent addition to the vocabualry of the Finnish language, and stands for sosiaali- ja terveydenhuolto 'social and health servives'. It is often used in connection with an ongoing discussion about plans to revamp the existing system of social and health services.
