Vergleichende Untersuchung der regenerationsfördernden Wirkung von Physiogel Creme auf die epidermale Barrierefunktion im repetitiven Irritationstest mit Natriumlaurylsulfat an hautgesunden Probanden

Zusammenfassung
Die Barriere-regenerierende Wirkung des Produktes Physiogel Creme, Stiefel Laboratorium GmbH, wurde im Vergleich zu kommerziellen Vergleichspräparaten und zu Vaseline DAB in einem kumulativen Irritationsmodell an hautgesunden Probanden in vivo überprüft.
Bei 20 Probanden wurde im Bereich der Unterarme eine kumulative Irritation mittels Applikation von 0.5% Natriumlaurylsulfat (SLS) zweimal täglich über vier Tage durchgeführt. Nach den Irritationen erfolgte jeweils die Applikation der Prüfpräparate. Die Hautreaktionen wurden klinisch mittels des Irritations-Scores nach Frosch und Kligman und hautphysiologisch mittels der Messung des transepidermalen Wasserverlustes (TEWL), der Hautrötung und der Hautfeuchtigkeit beurteilt.
Auf dem irritierten, aber ansonsten unbehandelten Kontrollfeld kam es erwartungsgemäss zu einer irritativen Ekzemreaktion, die sich in einer Erhöhung des visuellen Scores, des TEWL, der Hautrötung und der Hautfeuchtigkeit zeigte. Keines der Prüfpräparate konnte die Irritationsreaktion signifikant beeinflussen. Lediglich die Applikation von Vaseline führte zu einer Verstärkung der Reaktion im visuellen Score, was durch den okklusiven Effekt zu erklären sein dürfte.
Zusammenfassend konnte in der vorliegenden Untersuchung keine signifikante Barriere-regenerierende Wirkung von Physiogel Creme, jedoch auch nicht der Vergleichspräparate gezeigt werden. Dies könnte darauf zurückzuführen sein, dass Physiogel Creme ihre Wirkung bevorzugt bei trockener Haut entfaltet, was erst nach längerer Induktion eines irritativen Ekzems gegeben ist.
1. Einleitung
Unser Hautorgan ist zahlreichen Umwelteinflüssen, einer Vielzahl von Pflegemaßnahmen und -Produkten sowie beruflich bedingten Hautbelastungen ausgesetzt, die eine zunehmende Empfindlichkeit und Trockenheit der Haut begünstigen können. Bei entsprechender Disposition für eine trockene Haut, z. B. bei atopischer Hautdiathese, wirkt sich dies besonders nachhaltig aus. Nicht nur in Berufsfeldern mit hohem Anteil an Feuchtarbeiten, sondern auch im Privatleben kann es beispielsweise durch Hausarbeit zu einer Summation unterschwellig schädigender Reizeinwirkungen auf das Hautorgan kommen, die eine irritative Schädigung der Haut im Sinne eines subtoxisch-kumulativen Kontaktekzems bewirken kann, das sich im Falle beruflicher Einflüsse vorwiegend an den Händen manifestiert. Aber auch die Gesichtshaut kann durch Umwelteinflüsse und Anwendung von Pflegeprodukten irritativ geschädigt werden. Insofern ist eine geeignete Hautpflege, die selbst nicht irritierend wirkt und die physiologische Barrireregeneration der Epidermis zu unterstützen vermag, von herausragender Bedeutung. In zahlreichen Untersuchungen konnte die regenerationsfördernde Wirkung von Pflegeprodukten mit geeigneter Zusammensetzung bei geschädigter Barrierefunktion bereits belegt werden 1,2.
Die intakte Barrierefunktion wird durch die epidermale Lipidbarriere in der Hornschicht der Epidermis aufrechterhalten und bildet die wesentliche physiologische Schutzfunktion zur Umwelt. Endogene und /oder exogene Störungen spielen eine wesentliche Rolle bei der Pathogenese der trockenen Haut und daraus resultierender Dermatosen. Herkömmliche O/W- oder W/O-Emulsionen in Hautpflegepräparaten erfordern die Verwendung von Emulgatoren. Diese behalten ihre Emulgierfähigkeit auch beim Eindringen in die Haut bei und mizellieren dort die Lipide der Hornschicht. Dies kann zu einer ungewollten Beeinträchtigung der kontinuierlichen Doppelmembranstruktur damit der physiologischen Permeabilitätsbarriere führen 3.
De la Maza et al. 4 konnten zeigen, daß Emulgatoren, die eine den Hautlipiden ähnliche chemische Struktur besitzen, die Lipidbarriere weniger beeinträchtigen. Neuerdings kommen insofern alternativ zu O/W- bzw. W/O-Emulsionen z. B. Liposomen zum Einsatz, die wässrige Bestandteile direkt mit natürlichen Lipidbestandteilen der Hornschicht verbinden. Das innovative Konzept sog. DMS-Cremes (DMS = Derma-Membrane-Structure) beruht auf dem Prinzip der Analogie zur physiologischen Lipid-Bilayerstruktur. Die enthaltenen Biomoleküle bilden in Wasser lamellare Doppelmembranen und besitzen damit ähnliche physikalische Eigenschaften wie die Lipide der Permeabilitätsbarriere.
In Physiogel Creme der Stiefel Laboratorium GmbH wurde das DMS-Konzept verwirklicht. Bei den Bestandteilen wie Triglyceriden, Cholesterin, Phospholipiden, freien Fettsäuren, Squalen und Ceramiden handelt es sich aussschließlich um hautverwandte Lipide pflanzlichen Ursprungs. Da keine künstlichen Emulgatoren zugesetzt sind, werden nach Einwirken des Externums in nachfolgenden Waschvorgängen die natürlichen Barrierelipide nicht aus der Hornschicht der Haut herausgewaschen 5. Hautverwandte Lipide und Ceramide sollen in die oberen Schichten der Epidermis eindringen, und können hier zu einer Aufrechterhaltung und Regeneration der Hautbarriere beitragen und der Austrocknung der Haut vorbeugen. Theoretisch ergibt sich durch dieses Konzept ein Vorteil in Bezug auf eine hautschützende und regenerationsfördernde Wirkung gegenüber herkömmlichen Pflegeprodukten auf O/W- bzw. W/O-Basis, den es in vergleichenden klinischen Studien unter standardisierten experimentellen Bedingungen mittels hautphysiologischer Parameter zu belegen gilt. In Studien zur Wirksamkeitsprüfung verschiedener Hautpflegeprodukte konnte an der intakten Haut bereits gezeigt werden, dass Physiogel Creme im Vergleich zu herkömmlichen O/W- bzw. W/O-Emulsionen signifikant die Hautrauhigkeit vermindert sowie die Hautfeuchtigkeit verbessert 6,7.
Zur Untersuchung der Wirksamkeit und Sicherheit von Hautschutz/-pflegepräparaten entwickelten Frosch et al. [8] ein Modell eines repetitiven Irritationstests (RIT). Das Model eignet sich auch zur Wirksamkeitsprüfung von regenerationsfördernden Pflegeprodukten, indem diese jeweils im Anschluss an die SLS-Irritation aufgetragen werden. Die Hautreaktion wird mit verschiedenen hautphysiologischen Untersuchungsmethoden gemessen und analysiert. In einer Reihe von Untersuchungen unserer Arbeitsgruppe kam der RIT modifiziert zur Anwendung [9-12].
2. Material und Methode
2.1. Studiendesign
Die klinische Prüfung wurde als einseitige Blindstudie an hautgesunden Probanden, die sich freiwillig für die Teilnahme an der Untersuchung bereit erklärten und ihr Einverständnis mündlich und schriftlich erklärt hatten, durchgeführt. Das Studienprotokoll von der Ethik-kommission der Friedrich-Schiller-Universität genehmigt.
2.2. Probandenauswahl
Es wurden 20 hautgesunde Probanden im Alter zwischen 18 und 40 Jahren (16 weibliche und 4 männliche Probanden, Median 30,6 /- 6,2) rekrutiert.
Im Studienzeitraum war den Probanden normales Duschen gestattet. Direkte Anwendung von Detergenzien und von Hautpflegeprodukten an beiden Unterarmen war untersagt. Ferner waren die Probanden gehalten, Sonnenbäder zu vermeiden.
Einschlusskriterien:

4. keine schweren internistischen Erkrankungen
5. keine schweren dermatologischen Erkrankungen
Ausschlusskriterien:
1. Probanden mit schweren internistischen Allgemeinerkrankungen
2. Immunsuppressive Therapie (Corticosteroide)
3. Schwangerschaft, Stillzeit und unsichere Antikonzeption
4. Dermatologische Lokaltherapie der Unterarme innerhalb d. letzten 14 Tage 5. Bekannte Typ-IV-Sensibilisierungen ggü. Inhaltsstoffen der Testprodukte
6. Mangelnde Kooperationsbereitschaft
2.3. Testsubstanzen/Testprodukte
Pro Testfeld und Irritation kamen je 50 µl Natriumlaurylsulfat (SLS) höchster Reinheit 0.5% in Aqua dest. (Merck, pH 7,2) zur Anwendung.

2.4. Untersuchungsablauf
Applikationsstellen und -modus
Applikationsareal war der volare Unterarm. Es wurden im Abstand von ca. 3 cm insgesamt 8 Testfelder (jeweils 2 cm Durchmesser) entsprechend der Zahl der Testprodukte einschließlich eines Kontrollfeldes (nur SLS) auf der Haut markiert.
Mittels großer Finn Chambers und Filterscheiben wurde das Irritans in Kammern auf die Testfelder aufgetragen. Die kumulative Irritation wurde durch Applikation von 50 ųl SLS 0,5 % in Aqua dest. okklusiv induziert. Der Applikationsmodus betrug 2x täglich jeweils 30 Minuten im Abstand von 3 Stunden an insgesamt 4 Tagen. Behandelt wurde an den Tagen 1 bis 4 (Montag bis Donnerstag) jeweils zur gleichen Tageszeit (± 1 h). Die Testpflaster wurden nach 30-minütiger Einwirkzeit abgenommen und die Felder mit Papiertüchern abgetupft.
Nach der Irritation wurden die Felder entsprechend einem Randomisierungsplan mit 0.05 ml der zu prüfenden Externa behandelt, ein Testfeld wurde nicht nachbehandelt (Negativkontrolle). Die Externa wurden 10 min nach der Irritation auf die Testfelder aufgetragen und erst nach 15 Minuten mit Kleidung bedeckt.
Klinische Beurteilung und Messungen
Alle klinischen Beurteilungen erfolgten vor der Irritation an den Tagen 1-4 sowie am Tag 5. Die Messungen wurden nach einem Zeitraum von mindestens 15 Minuten körperlicher Ruhe unter standardisierten klimatischen Laborbedingungen (Raumtemperatur 20-22 0C, RLF 35-40 %) vorgenommen. Es wurden folgende klinische Beurteilungen und Messungen durchgeführt: Irritationsscores nach Frosch und Kligman 13, Messung des transepidermalen Wasserverlusts (TEWL, Tewameter), der Hornschichtfeuchtigkeit (Corneometer) und der Hautfarbe (Parameter L*, a*, b*) (Chromameter).
Irritationsscore nach Frosch und Kligman
Erythem: 1 leichte Rötung, fleckförmig oder diffus

Transepidermaler Wasserverlust
Zur Messung des transepidermalen Wasserverlustes (TEWL) wurde das Tewameter (Fa. Courage & Khazaka, Köln) verwendet.
Die genaue Anwendung des Gerätes ist bei Pinnagoda ausführlich beschrieben und folgt den dortigen Angaben [14].
Hornschichtfeuchtigkeit
Zur Messung der Hornschichtfeuchtigkeit wurde das Corneometer (CM 820 PC der Firma. Courage & Khazaka, Köln) verwendet.
Farbreflektion
Zur Quantifizierung des Erythems wurde das Chromameter (Minolta) (a*-Wert) nach den Empfehlungen von Elsner verwendet [15].
2.5. Auswertung
Die Ergebnisse der Zielparameter wurden in Tabellen und Grafiken zusammengefasst und eine deskriptive Statistik gerechnet. Unterschiede zwischen den Zielgrößen an den verschiedenen Testarealen wurden mittels einer Varianzanalyse auf Signifikanz überprüft.
Die statistische Auswertung der Daten erfolgte mit dem Software Programm SPSS 10.0 für Windows. In die Endauswertung ging jeweils der Mittelwert von drei Einzelmessungen, im Falle des TEWL von zwei Einzelmessungen, ein. Die Mittelwerte wurden in Säulenhistogrammen zusammen mit den Standardfehlern der Mittelwerte dargestellt. Für den Vergleich der Zielgrößen zwischen den verschiedenen Testfeldern wurde der nicht-parametrische Wilcoxon-Test für verbundene Stichproben durchgeführt. Da multiple Paarvergleiche erforderlich waren, erfolgte eine Bonferroni-Korrektur. Die so errechneten Signifikanzen wurden wie folgt in den Abbildungen gekennzeichnet:* p 0,05; ** p 0,01; *** p 0,001.
3. Ergebnisse
Kein Proband beendete die Studie vorzeitig. Die Studienprodukte wurden von allen Probanden gut vertragen. Das Abbruchkriterium (visueller Score > 5) wurde bei keinem der Testfelder erreicht.
Visueller Score (Abb.1)
Auf dem ausschließlich irritierten Kontrollfeld kam es von Tag 1 zu Tag 5 zu einem leichten Anstieg auf einen Mittelwert von 0,75. Auch auf allen anderen mit den verschiedenen Testprodukten nachbehandelten Testarealen kam es zu einem Anstieg des visuellen Scores, der nach Physiogel Creme, Lipidiose II rückfettende Körpercreme, Linola Fett N und Vaseline DAB im Vergleich zur Kontrolle höher ausfiel. Allerdings war der Unterschied nach Bonferroni-Korrektur nur für Vaseline DAB signifikant. Nur die Produkte Asche Basis Creme und Ictyane light emulsion moisturizing führten im Vergleich zur Kontrolle zu einer m schwachen Verminderung des Scores im Sinne einer tendenziellen Verminderung der SLS-induzierten Irritation, welche jedoch nicht signifikant war.

Chromametrie (Abb. 2)
Die Ausgangswerte aller Testfelder an Tag 1 zeigten keine statistisch signifikanten Unterschiede. Bis zum Tag 5 kam es auf allen Testfeldern zu einem Anstieg der Hautrötung a*, der auf dem Kontrollfeld im Mittel 2,6 (SEM /- 0,8) betrug. Weder die Nachbehandlung mit Physiogel Creme, noch mit einem der Vergleichsprodukte, konnte die chromametrisch messbare Erythemausprägung im Vergleich zur Kontrolle signifikant mindern. Physiogel Creme führte zu einer diskreten, klinisch zu vernachlässigenden Erythemverstärkung. Deutlicher, jedoch ebenfalls nicht signifikant, war die Erythemverstärkung durch die die Produkte Linola Fett N und Vaseline DAB.

Tewametrie (Abb. 3)
Am Tag 1 lagen die Ausgangswerte im Mittel bei 10,0 g/qm/h und zeigten keine signifikanten Unterschiede zwischen den Testfeldern. Die Ergebnisse der Tewametrie an Tag 5 spiegeln diejenigen des visuellen Scores und der Chromametrie wider. Auf dem Kontrollfeld wurde ein mässiggradiger mittlerer TEWL-Anstieg von 11,8 g/qm/h induziert. Keines der Testprodukte vermochte die SLS-induzierte Barrierestörung signifikant zu vermindern. Die Nachbehandlung mit Physiogel Creme führte ebenso wie mit Linola Fett N und Vaseline DAB tendenziell zu einer Verstärkung der Barrierestörung, die allerdings ebenfalls nicht signifikant war im Vergleich zur Kontrolle.

Corneometrie (Abb. 4)
Durch die kumulative SLS-Irritation kam es auf dem Kontrollfeld wie auch auf den mit den Testprodukten nachbehandelten Testarealen zu einem signifikanten Anstieg der Hornschichtfeuchtigkeit von Tag 1 zu Tag 5, der auf dem Kontrollfeld am geringsten ausgeprägt war. Lediglich die Präparate Aqueo Hand and Body Creme, Lipidiose II rückfettende Körpercreme und Ictyane light emulsion moisturizing führten zu einer gegenüber dem Kontrollfeld signifikanten Steigerung der Hautfeuchtigkeit (nach Bonferroni-Korrektur). Auch Physiogel Creme wirkte sich steigernd auf die Hydratation aus, der Unterschied war aber nach Bonferroni-Korrektur nicht mehr signifikant im Vergleich zur Kontrolle.

4. Diskussion
Weder für Physiogel Creme noch für eines der Vergleichspräparate war anhand des visuellen Scores, der Chromametrie oder der Tewametrie ein signifikant positiver Effekt auf die natürliche Barriereregeneration nachzuweisen. Die Hornschichtfeuchtigkeit wurde von einzelnen der Vergleichsprodukte signifikant gesteigert werden. Dies ist im Rahmen der vorliegenden Untersuchung jedoch eher als eine Irritationsverstärkung zu werten, da auch die Hydratation auf dem ausschließlich SLS-irritierten Kontrollfeld von Tag 1 zu Tag 5 leicht anstieg.
Eine Überlegenheit des in Physiogel Creme verwirklichten DMS-Konzeptes in Bezug auf eine Beschleunigung der Barriererestitution im Vergleich zu herkömmlichen O/W bzw W/O-Emulsionen konnte in dieser Studie nicht demonstriert werden. Lediglich gegenüber Vaseline, die im visuellen Score die SLS-induzierte Irritation signifikant und in der Tewametrie tendenziell verstärkte, zeigte sich eine Überlegenheit aller Testprodukte. Dies könnte durch die okklusiven Eigenschaften der Vaseline erklärt werden, die offenbar die Barrierereparatur zu verzögern vermag, wie bereits kürzlich von unserer Arbeitsgruppe in einer anderen Studie beobachtet wurde 16. Möglicherweise spielen auch bei den hier untersuchten Produkten unerwünschte okklusive Effekte eine Rolle, die sich insbesondere bei zeitnahem Auftragen der Externa nach Irritation auswirken können. Zusätzlich ist zu bedenken, dass die induzierte Irritation in unserem Probandenkollektiv insgesamt nur leicht bis mittelgradig ausgeprägt war. Hierfür können sowohl eine individuell unterschiedliche, im untersuchten Kollektiv insgesamt offenbar verhältnismäßig gering ausgeprägte Empfindlichkeit der Probanden gegenüber einer SLS-Irritation, als auch klimatische Einflüsse (Frühjahr mit zunehmend milden Außentemperaturen) eine Rolle spielen.
Zusammenfassend kann festgestellt werden, dass sich in der vorliegenden Studie ein Vorteil für Physiogel Creme gegenüber den anderen Testprodukten in bezug auf die Barriereregeneration nach standardisierter SLS-Irritation anhand der verschiedenen hautphysiologischen Testverfahren nicht herausstellen ließ. Es ist nicht auszuschließen, dass sich Unterschiede zwischen den Prüfpräparaten erst nach längerer Anwendungsdauer oder nur in einem Studienmodell nachweisen lassen, in dem die Behandlung nach und nicht parallel zu einer Barriereschädigung erfolgt. So könnte Physiogel Creme seine Wirkung bevorzugt bei trockener Haut entfalten, was erst nach längerer Induktion eines irritativen Ekzems gegeben ist. Sinnvoll erscheint in diesem Zusammenhang eine weiterführende Anwendungsstudie über einen vierwöchigen Zeitraum an Probanden mit mildem manifestem atopischen Ekzem mit Anwendung der Physiogel Creme nach standardisierter Vorbehandlung und im Seitenvergleich.

Summary
The barrier-regenerating effects of the product Physiogel Cream, Stiefel Laboratories GmbH, were tested in vivo against those of commercial comparator products as well as Vaseline in a cumulative irritation model on subjects with healthy skin.
A cumulative irritation in the forearm region was induced in 20 subjects through twice-daily application of 0.5% sodium lauryl sulfate (SLS). Following irritation, the investigational products were applied. Skin reactions were clinically evaluated using an irritation score based on Frosch and Kligman, and physiologically evaluated by measuring transepidermal water loss (TEWL), erythema and skin moisture.
As expected, an irritative eczematous reaction, characterized by increased visual scores, TEWL, skin redness and moisture, was observed at the irritated but otherwise untreated control areas. None of the investigational products significantly influenced the reaction to irritation. Only the application of Vaseline resulted in an increase of the reaction judged by visual score, which could be due to an occlusive effect.
In summary, the investigation described above could not demonstrate any significant barrier-regenerating effect of either Physiogel Cream, or any of the other products compared. One possible explanation of these results is that the effects of Physiogel Cream are most pronounced on dry skin, which occurs only after fairly protracted induction of irritant eczema.
1. Introduction
Our skin is exposed to countless environmental effects, numerous skin care treatments and products, as well as occupational stresses, all of which can cause increased skin sensitivity and dryness. These effects are especially sustained in people predisposed to dry skin, such as those with atopic skin diathesis. An accumulation of subliminal skin-damaging stimuli can occur not only in wet work environments, but also at home while working around the house. This can lead to skin damage in the form of cumulative irritant dermatitis, which in the case of occupational exposure, usually manifests itself on the hands. Furthermore, irritants from environmental sources and skin care products can also damage facial skin. Thus, skin care which does not irritate but rather promotes barrier regeneration in the epidermis is of the utmost importance. Numerous studies have demonstrated that skin care products with the proper formulation can have such a regenerating effect 1,2.
Intact barrier function is maintained by the epidermal lipid barrier in the stratum corneum of the epidermis, and acts as the main physiological protection from the environment. Endogenous and/or exogenous disruptions play an important role in the pathogenesis of dry skin and the resulting dermatosis . Conventional o/w or w/o emulsions in skin care products require the use of emulsifiers, which maintain their emulsive properties even while penetrating the skin, where they micellate the lipids of the stratum stratum corneum. . This can lead to an undesirable disruption of the continuous double membrane structure, and with it a disruption of the physiological permeability barrier 3.
De la Maza et al. 4 showed that emulsifiers with a chemical structure similar to the skin lipids disturb the lipid barrier to a lesser extent. Recently, alternatives to o/w or w/o emulsions, such as liposomes, have been described which incorporate aqueous components directly with the natural lipid components of the stratum corneum . The innovative concept behind the so-called DMS Creams (DMS = Derma-Membrane-Structure) is based on the principle of its similarity to the structure of the physiological lipid bilayer. The biomolecules in the creams form lamellar double membranes in water, giving them physical properties similar to those of the lipids of the permeability barrier.
The DMS concept has been realised with Physiogel Cream from Stiefel Laboratories GmbH. The components, including triglycerides, cholesterols, phospholipids, free fatty acids, squalene and ceramides, are exclusively plant-derived lipids similar to those in the skin. Since no artificial emulsifiers are added, natural barrier lipids are not washed out of the stratum corneum after absorption of the topical substance and subsequent washing 5. Lipids and ceramides similar to those in the skin penetrate into the upper layers of the epidermis, where they contribute to the maintenance and regeneration of the skin barrier and prevent the skin from drying out. In theory, this should afford an advantage over conventional skin care products based on o/w or w/o emulsions with regard to a protective and regenerating effect, but the concept remains to be tested in comparative clinical studies under standard experimental conditions using physiological skin parameters. Studies of the effectiveness of various skin care products on intact skin have already shown that Physiogel Cream significantly decreases roughness of skin and increases skin moisture when compared with conventional o/w or w/o emulsions 6.7.
To test the effectiveness and safety of skin protection and skin care products, Frosch et al. [8] developed a repetitive irritation test (RIT) model. This model is also suitable for measuring the effectiveness of regenerative skin care products when they are applied following irritation with SLS. The reaction of the skin is measured and analysed using several physiological methods of examination. Our group used a series of experiments based on a modified RIT [9-12].
2. Materials and Methods
2.1. Study Design
The clinical study was carried out as a single-blind study on subjects with healthy skin, who participated voluntarily and gave oral and written consent. The study protocol was approved by the Ethics Commission of the Friedrich-Schiller-University.
2.2. Subject Selection
Twenty subjects with healthy skin between the ages of 18 and 40 (16 female and 4 male subjects, median age 30.6 /- 6.2) were recruited.
Subjects were allowed to shower normally for the duration of the study. Direct application of detergents and skin care products on both forearms was prohibited. Subjects were further asked to avoid sun tanning.
Inclusion criteria:

4. No serious internal illnesses
5. No serious dermatological illnesses
Exclusion criteria:
1. Subjects with serious general internal illnesses
2. Immunosuppressant therapy (corticosteroids)
3. Pregnancy, lactation and unreliable contraception
4. Local dermatological therapy on the forearms during the previous 14 days
5. Known type IV hypersensitivity to contents of the tested products
6. Lack of willingness to cooperate
2.3. Test Substances/Test Products
50 µl of 0.5% highest purity sodium lauryl sulfate (SLS) in aqua dest. (Merck, pH 7.2) were used per test area

2.4. Test procedures
Site and mode of application
Area of application was the volar forearm. A total of 8 test areas (each 2 cm in diameter), 3 cm apart, equal to the number of test products as well as one control area (SLS only), were marked on the skin.
The irritant was applied to the test areas using large Finn Chambers and filter discs. The cumulative irritation was occlusively induced by applying 50 µl of 0.5 % SLS in aqua dest. Application was performed twice daily for 30 minutes with an interval of 3 hours on a total of 4 days. The treatment was applied on days 1 to 4 (Monday to Thursday) at the same time every day (± 1 h). Test patches were removed after 30 minutes of absorption, and the areas were dried with paper towels.
After irritation, the areas were treated with 0.05 ml of the topical products on a randomised basis. One test area was not treated (negative control). The topical substances were applied to the test areas 10 min after irritation, and were not covered with clothing for 15 minutes.
Clinical Evaluation and Measurements
All clinical evaluations were carried out before irritation on days 1-4 as well as on day 5. Measurements were taken after at least 15 minutes of rest at standard lab climate conditions (room temperature 20-22 0C, RH 35-40 %). The following clinical evaluations and measurements were performed: Irritation score based on the method of Frosch and Kligman 13, measurements of transepidermal water loss (TEWL, Tewameter), stratum corneum moisture (Corneometer) and skin colour (Parameter L*, a*, b*) (Chromameter).

Transepidermal Water Loss
To measure transepidermal water loss (TEWL), a Tewameter (Courage & Khazaka, Cologne) was used.
The precise use of the device has been described by Pinnagoda and was carried out according to the description therein [14].
Stratum corneumMoisture
Measurements of stratum corneum moisture were performed using a Corneometer (CM 820 PC from Courage & Khazaka, Cologne).
Colour Reflection
Quantification of erythema was carried out using a Chromameter (Minolta) (a*-Value) following the recommendations of Elsner [15].
2.5. Analysis
The results of the target parameters were summarized in tables and graphs, and descriptive statistics were calculated. Differences between target sizes of the various test areas were tested for significance using a variance analysis.
2.5.1 Statistical analyses were performed using the software program SPSS 10.0 for Windows. The final analysis was based on the mean value of three independent measurements, or two for TEWL. The mean values were depicted in bar graphs together with the standard deviation from the mean. To compare the target sizes between the test areas, the non-parametric Wilcoxon test for matched samples was used. Since multiple pair comparisons were required, a Bonferroni correction was performed. . The significant values calculated in this manner were labeled in the figures as follows:* p 0.05; ** p 0.01; *** p 0.001.
3. Results
No subject ended the study prematurely. The test products were well-tolerated by all subjects. The termination criterion (visual score > 5) was not achieved in any test areas.
Visual Score (Figure 1)
On the irritation-only control area, there was a slight increase to a mean value of 0.75 from day 1 to day 5. An increase in the visual score was also observed on all other test areas treated with the various products: Physiogel Cream, Lipidiose II Moisturising Body Cream, Linola Fett N and Vaseline were all higher than the control. However, following Bonferroni correction, the difference was significant only for Vaseline. By comparison with the control, the products Asche Basic Cream and Ictyane Emollient Crème were the only ones which led to a slight decrease in the scores, suggesting a tendential decrease in SLS-induced irritation, which however was not significant.

Chromametry (Figure 2)
There was no statistically significant difference in the initial values of any of the test areas on day 1. By day 5, all test areas showed increased redness a*, with a control area mean of 2.6 (SEM /- 0.8). Neither treatment with Physiogel Cream nor with any of the comparison products led to a significant decrease in the chromametrically measured degree of erythema compared to the control. Physiogel Cream led to discrete, clinically irrelevant erythema intensification. The erythema intensification with the products Linola Fett N and Vaseline was more pronounced but also not significant.

Tewametry (Figure 3)
On day 1, the mean of the initial values was 10.0 g/m2/h with no significant differences between the test areas. The tewametry results from day 5 reflect those of the visual score and chromametry. A moderate mean increase in TEWL of 11.8 g/m2/h was induced on the control area. None of the test products was able to significantly decrease the SLS-induced barrier modification . After-treatment with Physiogel Cream, as well as with Linola Fett N and Vaseline tended to lead to an increase in barrier modification, which also did not differ significantly from the control.

Corneometry (Figure 4)
As a result of the cumulative SLS irritation, there was a significant increase in stratum corneum moisture from day 1 to day 5 on the test areas after-treated with test products and also on the control area, where it was least pronounced. Only the products Aqueo Hand and Body Cream, Lipidiose II Moisturising Body Cream and Ictyane Emollient Crème led to a significant increase in skin moisture compared to the control area (after Bonferroni correction). Physiogel Cream also increased hydration; however, after Bonferroni correction, the difference was not significant compared to the control.

4. Discussion
Based on visual scores, chromametry, and tewametry, no significantly positive effect on natural barrier regeneration could be demonstrated for either Physiogel Cream or any of the products compared. Stratum corneum moisture was significantly increased by several of the products compared. However, in the context of the present examination, this result should be interpreted as an intensification of the irritation, since even hydration also increased slightly from day 1 to day 5 in the control area which was only irritated with SLS.
This study could not demonstrate the superiority of the DMS concept exemplified in Physiogel Creme in hastening barrier restitution compared to conventional o/w or w/o emulsions. The superiority of all test products was observed only over Vaseline, which demonstrated significantly increased SLS-induced irritation by visual score and a slight increase by tewametry. This may be explained by the occlusive properties of Vaseline, which can apparently delay barrier repair, as has been recently observed by our group in another study 16. It is also possible that undesirable occlusive effects play a role in the products tested here, with effects being magnified by the application of topical agents soon after irritation. Furthermore, it should be noted that the degree of irritation induced in our subjects was generally only light to moderate. The relatively mild sensitivity of the subjects to SLS-irritation, which differed between individuals but was clear in the aggregate, could play a role here, as could the influence of the seasonal climate (spring with increasingly mild temperatures).
In summary, the current study could not demonstrate an advantage of Physiogel Cream over the comparison products in regard to barrier regeneration after standardised SLS irritation based on various physiological skin measurements. The possibility cannot be ruled out that differences between the tested substances become apparent only after longer periods of use, or in a study model in which treatment occurs after barrier disruption, and not parallel to it. Thus, the effects of Physiogel Cream may be more pronounced on dry skin, after a longer induction of irritative eczema. Based on this hypothesis, an additional four-week application study on subjects with mild atopic dermatitis manifestations would be an appropriate next step to test the use of Physiogel Cream following standardised preconditioning in a side-by-side test.

Wajde Ganim, in the eyes of many, is considered the conscience of the Muslim Brotherhood inside and out of Egypt. During the recent revolution of Cairo’s youth, he, like many fundamentalists, was quiet. He was quiet as the grave, expressing neither support nor opposition. However, with the arrival of Yusef Qardawi on the scene amidst throngs of youthful dreamers, Ganim and Saham and others appeared to become more vocal and more prominent, and to cast their votes in favor of a disfiguring change to the Egyptian Constitution.
Now they speak, with full openness and without hesitation, on the issues of democracy, liberty, the Constitution, popular sovereignty, general finance, civil society and the freedom to create political parties. They say, without any tact or diplomacy or false words (and that is good on their part), that their opinions represent the best beliefs and positions of religious groups everywhere. According to them, democracy essentially amounts to blasphemy. They have declared this as policy, telling members of the Brotherhood that anyone desirous of anything other than the Muslim nation extolled by the Brotherhood are blasphemers because democracy, fundamentally, is itself blasphemy! They deny that there is such a thing as a freedom of religion or of thought, rather asserting that there must be “limits to apostasy”! They deny further the right to form parties, the existence of public money, for the proper owner of all monies is Allah, not the people.
They say the diligent Muslim should seek to return to religion and to strive for the sake of it.
None of this is propaganda or exaggeration or a deviation from what is known as the genuine belief of the Muslim Brotherhood or their positions on democracy, freedom of thought, and the formation of parties. Is that what the people really want? Is Egypt really about to substitute the dictatorship of Mubarak for a dictatorship of the Brotherhood?

Greetings! I never thought I'd become a linguist until finances (and the gloom of the subject matter!) forced me to give up my attempt at an MA in 20th Centrury German History as a postgraduate student in the UK in 2005. As I scrambled for work, I chanced upon the field of freelance translation. Since then, language has become my sole focus.
I learned German prior to embarking on this translation path. Shortly after the events of 9/11 (but entirely unrelated to them!), I attended the University of Vienna, in Austria, with the aim of perfecting my German. I graduated my specialized course in 2002 with a Certificate of German Proficiency. Returning to the States, I resumed my university studies at Western Washington University in Bellingham, WA where I focused on History and German. In 2005 I graduated with a BA in German and in History. I also completed the requirements for a German Business Certificate (ZDfB) from the Goethe Institute.
I was a US Army linguist for five years between March 2007 and March 2012. German not being much in military demand nowadays, I had the fortune of attending Defense Language Institute in beautiful Monterey, CA, where I studied Modern Standard Arabic. After graduation from DLI, I worked directly for Intelligence customers translating a wide-range of classified documents, from personal letters, to news items, to technical manuals.
I have maintained native level fluency in Arabic since that time, attaining DLPT scores of 4/3 and ILR scores of 3+/3+/3. For those unfamiliar with US Department of Defense testing procedures, my scores approximate what most native university students could score. I also maintained my German during this time through personal study and periodic freelance collaborations.
I worked knowing that lives depended on my accuracy and timeliness. A missed word in my text could translate into disaster on far-off city streets. A missed deadline could mean empty chairs at a dinner table. Although the stakes are now different, my approach is not. I take pride in precision and punctuality.
But this is also fun! I take joy in unraveling the puzzle of language. How can anyone succeed in this field if they don't? Now that I am committed full-time to freelance translating, I plan to enjoy it.
