4. CLINICAL PARTICULARS
4.1 Therapeutic indications
Treatment of hypertension in adults.
Ramipril/Amlodipine Adamed is indicated as substitution therapy of patients with blood pressure adequately controlled with ramipril and amlodipine given concurrently at the same dose level.
4.2 Posology and method of administration
Posology
Ramipril/Amlodipine Adamed should not be used for initiating treatment of hypertension. The doses of each component should be individualised according to the patient profile and blood pressure control.
If a dose change is required, the dosing regimen should be individually determined using the individual components of ramipril and amlodipine first, and once established, it could be changed to Ramipril/Amlodipine Adamed.
The recommended dose is one capsule daily. The maximum daily dose is one capsule 10 mg/10 mg.
Special populations
Renal impairment
To find the optimal starting dose and maintenance dose of patients with renal impairment, the patients should be individually titrated using the individual components of amlodipine and ramipril.
Ramipril is slightly dialysable, the medicinal product should be administered few hours after haemodialysis is performed.
Amlodipine is not dialysable. Amlodipine should be administered with particular caution to patients undergoing dialysis.
Renal function and serum potassium levels should be monitored during therapy with Ramipril/Amlodipine Adamed. In the case of renal function deterioration, the use of Ramipril/Amlodipine Adamed should be discontinued and replaced by the individual components adequately adjusted.
Hepatic impairment
The maximum daily dose is 2.5 mg ramipril.
Elderly
Lower initial dosage is recommended in the elderly and increase of the dosage should take place with care.
Paediatric population
The safety and efficacy of Ramipril/Amlodipine Adamed in children has not been established.
Currently available data are described in section 4.8, 5.1, 5.2 and 5.3 but no recommendation on a posology can be made.
Method of administration
Since food does not affect absorption of ramipril and amlodipine, Ramipril/Amlodipine Adamed may be taken irrespective of meals. It is recommended that Ramipril/Amlodipine Adamed is taken at the same time of the day.
4.3 Contraindications
Hypersensitivity to ramipril, amlodipine, other ACE (Angiotensin Converting Enzyme) inhibitors, dihydropyridine derivatives or to any of the excipients listed in section 6.1.
Related to Ramipril
- Concomitant use of Ramipril/Amlodipine Adamed with aliskiren-containing products is contraindicated in patients with diabetes mellitus or renal impairment (GFR) 70 years), uncontrolled diabetes mellitus, or those using potassium salts, potassium retaining diuretics and other plasma potassium increasing active substances, or conditions such as dehydration, acute cardiac decompensation, metabolic acidosis. If concomitant use of the above mentioned agents is deemed appropriate, regular monitoring of serum potassium is recommended (see section 4.5).
Neutropenia/agranulocytosis
Neutropenia/agranulocytosis, as well as thrombocytopenia and anaemia, have been rarely seen and bone marrow depression has also been reported. It is recommended to monitor the white blood cell count to permit detection of a possible leucopoenia. More frequent monitoring is advised in the initial phase of treatment and in patients with impaired renal function, those with concomitant collagen disease (e.g. lupus erythematosus or scleroderma), and all those treated with other medicinal products that can cause changes in the blood picture (see sections 4.5 and 4.8).
Ethnic differences
ACE inhibitors cause higher rate of angioedema in black patients than in non black patients. As with other ACE inhibitors, ramipril may be less effective in lowering blood pressure in black people than in non black patients, possibly because of a higher prevalence of hypertension with low renin level in the black hypertensive population.
Cough
Cough has been reported with the use of ACE inhibitors. Characteristically, the cough is nonproductive, persistent and resolves after discontinuation of therapy. ACE inhibitor-induced cough should be considered as part of the differential diagnosis of cough.
Related to Amlodipine
The safety and efficacy of amlodipine in hypertensive crisis has not been established.
Special populations
Patients with cardiac failure
Patients with heart failure should be treated with caution. In a long-term, placebo controlled study in patients with severe heart failure (NYHA class III and IV) the reported incidence of pulmonary oedema was higher in the amlodipine treated group than in the placebo group (see section 5.1). Calcium channel blockers, including amlodipine, should be used with caution in patients with congestive heart failure, as they may increase the risk of future cardiovascular events and mortality.
Patients with impaired hepatic function
The half life of amlodipine is prolonged and AUC values are higher in patients with impaired liver function; dosage recommendations have not been established. Amlodipine should therefore be initiated at the lower end of the dosing range and caution should be used, both on initial treatment and when increasing the dose. Slow dose titration and careful monitoring may be required in patients with severe hepatic impairment.
Elderly patients
In the elderly increase of the dosage should take place with care (see sections 4.2 and 5.2).
4.5 Interaction with other medicinal products and other forms of interaction
Related to Ramipril
Contra-indicated combinations
Clinical trial data has shown that dual blockade of the renin-angiotensin-aldosterone-system (RAAS) through the combined use of ACE-inhibitors, angiotensin II receptor blockers or aliskiren is associated with a higher frequency of adverse events such as hypotension, hyperkalaemia and decreased renal function (including acute renal failure) compared to the use of a single RAAS-acting agent (see sections 4.3, 4.4 and 5.1).
Extracorporeal treatments leading to contact of blood with negatively charged surfaces such as dialysis or haemofiltration with certain high-flux membranes (e.g. polyacrylonitril membranes) and low density lipoprotein apheresis with dextran sulphate due to increased risk of severe anaphylactoid reactions (see section 4.3). If such treatment is required, consideration should be given to using a different type of dialysis membrane or a different class of antihypertensive agent.
Precautions for use
Potassium salts, heparin, potassium-retaining diuretics and other plasma potassium increasing active substances (including Angiotensin II antagonists, trimethoprim, tacrolimus, ciclosporin): Hyperkalaemia may occur, therefore close monitoring of serum potassium is required.
Antihypertensive agents (e.g. diuretics) and other substances that may decrease blood pressure (e.g. nitrates, tricyclic antidepressants, anaesthetics, acute alcohol intake, baclofen, alfuzosin, doxazosin, prazosin, tamsulosin, terazosin): Potentiation of the risk of hypotension is to be anticipated (see section 4.2 for diuretics)
Vasopressor sympathomimetics and other substances (e.g. isoproterenol, dobutamine, dopamine, epinephrine) that may reduce the antihypertensive effect of ramipril: Blood pressure monitoring is recommended.
Allopurinol, immunosuppressants, corticosteroids, procainamide, cytostatics and other substances that may change the blood cell count: Increased likelihood of haematological reactions (see section 4.4).
Lithium salts: Excretion of lithium may be reduced by ACE inhibitors and therefore lithium toxicity may be increased. Lithium level must be monitored.
Antidiabetic agents including insulin: Hypoglycaemic reactions may occur. Blood glucose monitoring is recommended.
Non-steroidal anti-inflammatory drugs and acetylsalicylic acid: Reduction of the antihypertensive effect of ramipril is to be anticipated. Furthermore, concomitant treatment of ACE inhibitors and NSAIDs may lead to an increased risk of worsening of renal function and to an increase in kalaemia.
Related to Amlodipine
Effects of other medicinal products on amlodipine
CYP3A4 inhibitors: Concomitant use of amlodipine with strong or moderate CYP3A4 inhibitors (protease inhibitors, azole antifungals, macrolides like erythromycin or clarithromycin, verapamil or diltiazem) may give rise to significant increase in amlodipine exposure. The clinical translation of these PK variations may be more pronounced in the elderly. Clinical monitoring and dose adjustment may thus be required.
CYP3A4 inducers: There is no data available regarding the effect of CYP3A4 inducers on amlodipine. The concomitant use of CYP3A4 inducers (e.g., rifampicin, hypericum perforatum) may give a lower plasma concentration of amlodipine. Amlodipine should be used with caution together with CYP3A4 inducers.
Administration of amlodipine with grapefruit or grapefruit juice is not recommended as bioavailability may be increased in some patients resulting in increased blood pressure lowering effects.
Dantrolene (infusion): In animals, lethal ventricular fibrillation and cardiovascular collapse are observed in association with hyperkalaemia after administration of verapamil and intravenous dantrolene. Due to risk of hyperkalemia, it is recommended that the co-administration of calcium channel blockers such as amlodipine be avoided in patients susceptible to malignant hyperthermia and in the management of malignant hyperthermia.
Effects of amlodipine on other medicinal products
The blood pressure lowering effects of amlodipine adds to the blood pressure-lowering effects of other medicinal products with antihypertensive properties.
In clinical interaction studies, amlodipine did not affect the pharmacokinetics of atorvastatin, digoxin, warfarin or cyclosporin.
Simvastatin: Co-administration of multiple doses of 10 mg of amlodipine with 80 mg simvastatin resulted in a 77% increase in exposure to simvastatin compared to simvastatin alone. Limit the dose of simvastatin to 20 mg daily in patients on amlodipine.
4.6 Fertility, pregnancy and lactation
Given the effects of the individual components in this combination product on pregnancy and lactation:
Ramipril/Amlodipine Adamed is not recommended during the first trimester of pregnancy and is contraindicated during the second and third trimesters of pregnancy.
Ramipril/Amlodipine Adamedis not recommended during lactation. A decision on whether to continue/discontinue breastfeeding or to continue/discontinue therapy with Ramipril/Amlodipine Adamed should be made taking into account the benefit of breastfeeding to the child and the benefit of amlodipine therapy to the mother.
Pregnancy
Related to Ramipril
The use of ACE inhibitors is not recommended during the first trimester of pregnancy (see section 4.4). The use of ACE inhibitors is contraindicated during the second and third trimesters of pregnancy (see sections 4.3 and 4.4).
Epidemiological evidence regarding the risk of teratogenicity following exposure to ACE inhibitors during the first trimester of pregnancy has not been conclusive; however a small increase in risk cannot be excluded. Unless continued ACE inhibitor therapy is considered essential, patients planning pregnancy should be changed to alternative anti-hypertensive treatments which have an established safety profile for use in pregnancy. When pregnancy is diagnosed, treatment with ACE inhibitors should be stopped immediately, and, if appropriate, alternative therapy should be started.
Exposure to ACE inhibitor therapy during the second and third trimesters is known to induce human foetotoxicity (decreased renal function, oligohydramnios, skull ossification retardation) and neonatal toxicity (renal failure, hypotension, hyperkalaemia) (see section 5.3). Should exposure to ACE inhibitors have occurred from the second trimester of pregnancy, ultrasound check of renal function and skull is recommended. Infants whose mothers have taken ACE inhibitors should be closely observed for hypotension, oliguria and hyperkalaemia (see sections 4.3 and 4.4).
Related to Amlodipine
The safety of amlodipine in human pregnancy has not been established.
In animal studies, reproductive toxicity was observed at high doses (see section 5.3).
Use in pregnancy is only recommended when there is no safer alternative and when the disease itself carries greater risk for the mother and foetus.
Breast-feeding
Related to Ramipril
Because insufficient information is available regarding the use of ramipril during breastfeeding (see section 5.2), ramipril is not recommended and alternative treatments with better established safety profiles during breast-feeding are preferable, especially while nursing a newborn or preterm infant.
Related to Amlodipine
It is not known whether amlodipine is excreted in breast milk. A decision on whether to continue/discontinue breastfeeding or to continue/discontinue therapy with amlodipine should be made taking into account the benefit of breastfeeding to the child and the benefit of amlodipine therapy to the mother.
Fertility
Reversible biochemical changes in the head of spermatozoa have been reported in some patients treated by calcium channel blockers. Clinical data are insufficient regarding the potential effect of amlodipine on fertility. In one rat study, adverse effects were found on male fertility (see section 5.3).
4.7 Effects on ability to drive and use machines
Ramipril/Amlodipine Adamed can have minor or moderate influence on the ability to drive and use machines. Some adverse effects (e.g. symptoms of a reduction in blood pressure such as dizziness, headache, fatigue) may impair the patient's ability to concentrate and react and, therefore, constitute a risk in situations where these abilities are of particular importance (e.g. operating a vehicle or machinery).
This can happen especially at the start of treatment, or when changing over from other preparations. Caution is recommended especially at the start of treatment.
4.8 Undesirable effects
The safety profile of ramipril includes persistent dry cough and reactions due to hypotension. Serious adverse reactions include stroke, myocardial infarction, angioedema, hyperkalaemia, renal or hepatic impairment, pancreatitis, severe skin reactions and neutropenia/agranulocytosis.
The most commonly reported adverse reactions during treatment with amlodipine are somnolence, dizziness, headache, palpitations, flushing, abdominal pain, nausea, ankle swelling, oedema and fatigue.
Adverse reactions frequency is defined using the following convention:

4.9 Overdose
Related to Ramipril
Symptoms associated with overdosage of ACE inhibitors may include excessive peripheral vasodilatation (with marked hypotension, shock), bradycardia, electrolyte disturbances, and renal failure. The patient should be closely monitored and the treatment should be symptomatic and supportive. Suggested measures include primary detoxification (gastric lavage, administration of adsorbents) and measures to restore haemodynamic stability, including, administration of alpha 1 adrenergic agonists or angiotensin II (angiotensinamide) administration. Ramiprilat, the active metabolite of ramipril is poorly removed from the general circulation by haemodialysis.
Related to Amlodipine
In humans experience with intentional overdose is limited.
Symptoms
Available data suggest that gross overdosage could result in excessive peripheral vasodilatation and possibly reflex tachycardia. Marked and probably prolonged systemic hypotension up to and including shock with fatal outcome have been reported.
Treatment
Clinically significant hypotension due to amlodipine overdosage calls for active cardiovascular support including frequent monitoring of cardiac and respiratory function, elevation of extremities, and attention to circulating fluid volume and urine output.
A vasoconstrictor may be helpful in restoring vascular tone and blood pressure, provided that there is no contraindication to its use. Intravenous calcium gluconate may be beneficial in reversing the effects of calcium channel blockade.
Gastric lavage may be worthwhile in some cases. In healthy volunteers the use of charcoal up to 2 hours after administration of amlodipine 10 mg has been shown to reduce the absorption rate of amlodipine. Since amlodipine is highly protein-bound, dialysis is not likely to be of benefit.

Liều khuyến cáo là một viên mỗi ngày. Liều tối đa hàng ngày là một viên nang 10 mg/10 mg.
Những nhóm bệnh nhân đặc biệt
Suy thận
Để xác định liều khởi đầu và liều duy trì tối ưu cho các bệnh nhân suy thận, cần tiến hành phân tích chuẩn độ cho mỗi bệnh nhân bằng cách sử dụng các thành phần riêng lẻ của amlodipin và ramipril.
Ramipril có thể bị phân tách ở mức độ không đáng kể, các dược phẩm nên được dùng vài giờ sau khi thực hiện thẩm phân máu.
Amlodipin không bị phân tách. Nên đặc biệt thận trọng khi dùng amlodipin đối với các bệnh nhân đang điều trị thẩm phân.
Chức năng thận và nồng độ kali huyết thanh cần được theo dõi khi điều trị bằng Ramipril/Amlodipin Adamed. Trong trường hợp suy giảm chức năng thận, nên ngưng dùng Ramipril/Amlodipin Adamed và thay bằng các thành phần riêng lẻ được điều chỉnh phù hợp.
Suy gan
Liều tối đa hàng ngày là 2,5 mg ramipril.
Người cao tuổi
Khuyến nghị nên sử dụng liều khởi đầu thấp hơn đối với người cao tuổi và cần phải thận trọng khi tăng liều lượng.
Nhóm bệnh nhân vị thành niên
Tính an toàn và hiệu quả của Ramipril/Amlodipin Adamed ở trẻ em chưa được kiểm chứng.
Các dữ liệu hiện có được mô tả trong phần 4.8, 5.1, 5.2 và 5.3 nhưng chưa có liều khuyến cáo nào được đưa ra.
Cách dùng
Vì thức ăn không ảnh hưởng đến sự hấp thụ của ramipril và amlodipine, có thể uống Ramipril/Amlodipin Adamed trong hoặc ngoài bữa ăn. Thuốc Ramipril/Amlodipin Adamed được khuyến cáo uống tại cùng một thời điểm trong ngày.
4.3 Chống chỉ định
Quá mẫn với ramipril, amlodipin, các thuốc ức chế ACE(Angiotensin Converting Enzyme - Men chuyển Angiotensin) khác, dẫn xuất dihydropyridine hay bất kỳ tá dược được liệt kê trong phần 6.1.
Liên quan đến Ramipril
- Việc sử dụng đồng thời Ramipril/Amlodipin Adamed với các sản phẩm có chứa Aliskiren được chống chỉ định đối với bệnh nhân đái tháo đường hoặc suy thận (GFR) 70 tuổi), đái tháo đường không kiểm soát, hoặc những người sử dụng các muối kali, thuốc lợi tiểu trừ miễn kali và các hoạt chất làm tăng kali huyết tương khác, hoặc các bệnh lý như mất nước, suy tim mất bù cấp, nhiễm toan chuyển hóa. Nếu sử dụng đồng thời các tác nhân nêu trên được xem là thích hợp, chúng tôi khuyến cáo nên theo dõi thường xuyên mức kali huyết thanh (xem phần 4.5).
Giảm bạch cầu trung tính/mất bạch cầu hạt
Chứng giảm bạch cầu trung tính/mất bạch cầu hạt, cũng như chứng giảm tiểu cầu và thiếu máu, rất ít khi bắt gặp và chứng suy tủy xương cũng đã được ghi nhận. Ho

Những nhóm bệnh nhân đặc biệt
Bệnh nhân suy tim
Bệnh nhân bị suy tim cần được điều trị cẩn trọng. Trong một nghiên cứu dài hạn có đối chứng với giả dược ở các bệnh nhân suy tim nặng (NYHA III và IV) tỷ lệ phù phổi được ghi nhận ở nhóm dùng amlodipine cao hơn so với nhóm dùng giả dược (xem phần 5.1). Thuốc chẹn kênh canxi, bao gồm amlodipine, nên được sử dụng thận trọng ở các bệnh nhân suy tim sung huyết vì có thể làm tăng nguy cơ mắc các cơn tai biến tim mạch và tử vong trong tương lai.
Bệnh nhân bị suy chức năng gan
Thời gian bán phân hủy của amlodipine bị kéo dài và các giá trị AUC cao hơn ở những bệnh nhân bị suy giảm chức năng gan; chưa có liều khuyến cáo. Do đó nên bắt đầu dùng Amlodipin ở các mức liều thấp và cần thận trọng, cả ở giai đoạn điều trị ban đầu và khi bắt đầu tăng liều lượng. Có thể cần phải tiến hành phân tích chuẩn độ và theo dõi liều lượng cẩn thận ở các bệnh nhân suy gan nặng.
Bệnh nhân cao tuổi
Đối với bệnh nhân cao tuổi, việc tăng liều lượng cần phải hết sức cẩn trọng (xem phần 4.2 và 5.2).
4.5 Tương tác với các thuốc khác và các dạng tương tác khác
Liên quan đến Ramipril
Kết hợp chống chỉ định
Dữ liệu thử nghiệm lâm sàng cho thấy việc ức chế kép hệ renin-angiotensin-aldosterone (RAAS) thông qua việc sử dụng kết hợp các thuốc ức chế ACE, chất ức chế thụ thể angiotensin II hoặc Aliskiren thường kéo theo tần suất xảy ra các tác dụng phụ cao hơn như tụt huyết áp, tăng kali máu và suy giảm chức năng thận (bao gồm cả suy thận cấp) so với việc sử dụng duy nhất một tác nhân tác động trực tiếp lên hệ RAAS (xem phần 4.3, 4.4 và 5.1).

Khuyến cáo khi sử dụng
Các muối kali, heparin, thuốc lợi tiểu giữ kali và các hoạt chất tăng kali huyết tương khác (bao gồm cả các chất đối kháng Angiotensin II, trimethoprim, tacrolimus, ciclosporin): Chứng tăng kali máu có thể xảy ra, do đó cần theo dõi sát sao lượng kali huyết thanh.

Thuốc ức chế acid uric allopurinol, thuốc ức chế miễn dịch, thuốc chống viêm corticosteroid, thuốc chống loạn nhịp procainamide, thuốc ức chế tăng trưởng tế báo cytostatics và các chất khác có thể thay đổi số lượng tế bào máu: Tăng khả năng phản ứng huyết học (xem phần 4.4).
Muối lithium: Các thuốc ức chế ACE có thể làm giảm mức độ bài tiết lithium và do đó có thể tăng nguy cơ ngộ độc lithium. Mức lithium cần phải được theo dõi sát sao.
Các thuốc chữa đái đường có chứa insulin: Có thể xảy ra các phản ứng hạ đường huyết. Khuyến cáo cần theo dõi lượng đường trong máu.
Các thuốc chống viêm không steroid và acid acetylsalicylic: Có thể dự kiến tác dụng hạ huyết áp của ramipril sẽ bị giảm đi. Hơn nữa, việc điều trị đồng thời các thuốc ức chế ACE và thuốc chống viêm không steroid (NSAID) có thể dẫn đến tăng nguy cơ suy giảm chức năng thận và tăng kali huyết.

Các chất ức chế CYP3A4: Việc sử dụng đồng thời amlodipine với các chất ức chế CYP3A4 mạnh hoặc trung bình (các thuốc ức chế men protease, thuốc kháng nấm nhóm azole, thuốc kháng sinh nhóm macrolid như erythromycin hoặc clarithromycin, verapamil hoặc diltiazem) có thể dẫn đến việc gia tăng đáng kể mức độ phơi nhiễm amlodipine. Cần thận trọng khi sử dụng Amlodipin với các thuốc gây cảm ứng CYP3A4.
Khuyến cáo không sử dụng amlodipine cùng với bưởi hoặc nước bưởi vì sinh khả dụng của thuốc có thể tăng lên ở một số bệnh nhân dẫn đến làm tăng nguy cơ hạ huyết áp.
Thuốc giãn cơ dantrolene (tiêm truyền): Ở động vật, rung tâm thất và trụy tim mạch gây tử vong được ghi nhận thường kèm theo chứng tăng kali máu sau khi sử dụng verapamil và dantrolene tiêm tĩnh mạch. Do nguy cơ tăng kali máu, chúng tôi khuyến cáo cần tránh dùng đồng thời các thuốc chẹn kênh canxi như amlodipine ở các bệnh nhân nhạy cảm với chứng tăng thân nhiệt ác tính và liệu pháp kiểm soát chứng tăng thân nhiệt ác tính.
Ảnh hưởng của amlodipine đối với các dược phẩm khác
Tác dụng hạ huyết áp của amlodipin kết hợp với tác dụng hạ huyết áp của các dược phẩm khác có đặc tính chống cao huyết áp.
Trong các nghiên cứu tương tác về mặt lâm sàng, amlodipin không ảnh hưởng đến dược động học của atorvastatin, digoxin, warfarin hoặc cyclosporin.
Simvastatin: Việc uống đồng thời nhiều liều 10 mg amlodipine, trong đó có 80 mg simvastatin mỗi liều, làm gia tăng 77% mức độ phơi nhiễm với simvastatin so với khi chỉ dùng simvastatin riêng rẽ. Vì vậy cần giới hạn liều simvastatin ở mức 20 mg hàng ngày đối với các bệnh nhân đang điều trị bằng amlodipin.
4.6 Khả năng sinh sản, mang thai và cho con bú
Do ảnh hưởng của các thành phần riêng biệt trong sản phẩm kết hợp này đối với khả năng mang thai và cho con bú:
Khuyến cáo không sử dụng Ramipril/Amlodipin Adamed trong ba tháng đầu của thai kỳ và chống chỉ định trong ba tháng thứ hai và thứ ba của thai kỳ.
Khuyến cáo không sử dụng Ramipril/Amlodipin Adamedis khi đang cho con bú. Khi đưa ra quyết định về việc có nên tiếp tục/ngưng cho con bú hoặc tiếp tục/ngưng điều trị bằng Ramipril/Amlodipin Adamed, cầm cân nhắc giữa lợi ích của sữa mẹ cho trẻ em và lợi ích điều trị bằng amlodipin cho người mẹ.

Khuyến cáo không sử dụng các thuốc ức chế ACE trong ba tháng đầu tiên của thai kỳ (xem phần 4.4). Chống chỉ định sử dụng các thuốc ức chế ACE trong tháng thứ hai và thứ ba của thai kỳ (xem mục 4.3 và 4.4).

4.7 Tác dụng đối với khả năng lái xe và vận hành máy móc

Soga Plum Forest
Featuring some 35,000 white plum trees abloom, the Odawara Plum Festival begins on February 2 and ends on March 3.

Odawara Flower Garden
Beginning in mid-January and lasting until early March, the Odawara Flower Garden is home to 480 plum trees belonging to some 200 different varieties.
A local plum tree festival and numerous other events are held during this time, considered the prime viewing season.
